Registration / Payment Procedures
Follow the steps below to register and make your payment for Beach Fiesta 08. Please read all instructions carefully.
All teams are required to make payment upon submission of registration forms. The registration fees would be $10 per participant per sport for all of the 5 sports (not inclusive of Sentosa entrance fee).
Payment can be made at any time before the event, but do note that registration will not be confirmed until upon receipt of payment. Places are on a first-come-first-served basis, so do make payment as soon as possible.
Registration
1) Click the 'Registration Form' link on the right to download registration form.
Do note that the registration form is the same for all the 5 sports. However, if a team wishes to sign up for more than one sport, the team would have to fill up a seperate form for each sport.
2) Fill in registration form, and submit to us at the FASS Club Room. Click here to find out where we are.
Please ensure that all details are filled up clearly and correctly. Do remember to indicate the sport which you are signing up for, and ensure that you have the required number of players.
3) If you are unable to come down to submit your forms personally, kindly send a softcopy of your form to jeremyteocx@gmail.com .
For those submitting the softcopy forms, please kindly print and fill up the indemnity forms, and submit them to us on the day of the event. Alternatively, you may scan the individual forms and send them in.
Every participant is required to fill up one indemnity form each. The organizing committee reserve the rights to disallow the participation of any teams without indemnity forms.
Payment
1) You can personally come down to make payment at the FASS Club Room. Kindly look for Xiao Yan or Jeremy. You will be issued a receipt upon payment.
2) For those who are submitting their forms through e-mail, kindly do an ATM transfer or i-Banking transfer to the following account. POSB Savings Account, 198-14149-6. Please print a copy of the transaction receipt for us to verify your payment.
3) Once the transaction is done, kindly drop Xiao Yan an SMS at 9270 6851 to inform her of the amount and transaction.
4) Please kindly bring down your transaction receipt on the day of event. It would be required for registration on the day itself.
Kindly note that the organizing committee will not be held responsible for any lost or mis-handled transactions. It is strongly recommended that you come down personally to submit your forms and payment.
